Birth Certificate Long Form
The ‘long form’ certificate is a printed image of the original certificate of birth. In addition to the child’s name and date of birth, it contains information such as the name of the hospital, address of parents, parents’ dates of birth, etc. The certificate contains a raised seal which denotes certification. Registration time for birth record are 45 days from the date of event.
Death Certificate (without Cause)
A certified copy that does not include the cause of death. This certificate can typically be used to obtain death benefits, claim insurance proceeds, notify social security and other legal purposes. Registration time for death records are 30 days from the date of event.
Marriage Certificate
A certified copy of the Certificate of Marriage. This is not a copy of the marriage license. Registration time for marriage records are 30 days from the date of event.
Divorce Certificate
A certified copy of the Certificate of Divorce. This is not a copy of the divorce decree. Registration time for divorce records are 30 days from the date of event.
Birth Verification
A verification of an event that can typically be used for informational purposes only. This is not a certified document and is not intended for identification. It is a handwritten transcript of the information taken from the original certificate. Registration time for birth records are 45 days from the date of event.
Death Verification
A verification of the death event that can typically be used for informational purposes only. This is not a certified document and is not intended for legal purposes. The cause of death is not listed on the verification form. Registration time for death records are 30 days from the date of event.
