Identification Requirements
The Douglas County Health Department requires all applicants to submit a copy of proof of relationship (e.g. marriage certificate, birth certificate, court order) and a copy of their valid, government issued photo ID. You will be presented with an Authorization Form at the end of your order that includes specific instructions. When faxing your identification, please enlarge and lighten to ensure your fax is legible. Your order will not be processed until a legible form of identification and proof of relationship are received. Below is a list of acceptable forms of identification:

One of the following valid forms of identification (no expired documents are accepted):

  • Photo Driver's License, Photo ID Card (DMV), CO Temporary Driver's License Renewal Form (with hole-punched Driver's License), U.S. or Foreign Passport, U.S. Military ID Card, School University or College ID Card (must be current), U.S. Citizenship IF Card (I-197), Certificate of U.S. Citizenship, US. Certificate of Naturalization, U.S. B1/B2 Visa Card with I-94, Employment Authorization Card (I-766), Temporary Resident Card, Alien Registration Receipt/Permanent Resident Card, Colorado Department of Corrections ID Card, Resident Card, Government Work ID, Job Corps ID

Two of the following (any document expired more than six months will not be accepted):

  • Acknowledgement of Paternity, Birth Certificate of Applicant (U.S. only), Court order of adoption or name change, Craft or trade license (Colorado only), DD-214, Divorce Decree (U.S. only), Hospital birth worksheet (for infants under 6 mo.), Hunting or Fishing License (CO issued, must be current), IRS-TIN card, Marriage License (U.S. only), Medicare Card, Merchant Mariner Card, Mexican Voter Registration Card, Motor Vehicle Registration or Title (U.S. only), Pilot License, Social Security Card, Social Services Card (WIC check and WIC envelope), State or federal prison or corrections card, Tribal ID Card, Weapon or gun permit (U.S. only), Work ID, W-2, or paycheck stub (within 3 mo.), Any expired document from the 'Primary List' (cannot be expired more than 6 mo.)

If you cannot provide acceptable identification, it is suggested that you ask a spouse, parent, grandparent, sibling, or adult child, who can provide appropriate identification, to request the certificate. Proof of relationship is required, (e.g. marriage certificate, birth certificate, court order, etc.).
